Job summary

Volunteers of America Chesapeake and Carolinas is seeking a Community Support Worker in Washington, DC to conduct community support activities and assist consumers with rehabilitation goals under an individualized service plan. The role involves extensive community outreach and working in consumers’ natural settings.

Responsibilities include direct service, referrals, documentation, and collaboration with the Treatment Team.

Qualifications

• High School or GED required with 2 year of study or 3 years’ experience in a relevant human services field.
• Minimum 1 year of related human service activities/experience
• Bilingual English/Spanish fluent is required to serve Spanish-speaking demographic

Responsibilities

• Manage a caseload of approximately 40 adults
• Assist the consumer in developing interpersonal and community coping skills
• Assist in relapse and symptom management planning
• Prepare Individualized Service Plan with measurable objectives
• Provide and coordinate referrals for consumer needs
• Conduct visits to consumer's job site, home, or other settings
• Transport consumers as needed
• Maintain documentation per MHRS/VOAC standards
• Meet weekly with Treatment Team and provide progress notes
• Meet weekly/monthly productivity of 400 units
