CLUES is seeking a Community Resource Navigator in Saint Paul, MN, to connect Latino community members with needed services for mental health symptoms. The role includes bilingual outreach, education, and case management, coordinating with Ramsey County Human Services and CLUES teams.

The position requires a Bachelor’s degree in a related field, bilingual English/Spanish, and the ability to work a flexible schedule.

Formación

• Bachelor’s degree in social work, psychology, counseling, public health or related field or equivalent combination of education and experience required.
• Experience in providing case management services preferred.
• Experience providing outreach/engagement services in the community preferred.
• Experience working with individuals with symptoms of mental illness preferred.
• Bilingual in Spanish & English with cultural sensitivity and awareness required.
• Strong communications skills and ability to establish positive working relationships with clients/families, health care providers, and other involved community members required.
• Ability to work independently and as a member of a team required.
• Must be able to maintain a flexible schedule – generally Monday through Friday office hours, but some evenings and weekends may be necessary.
• Must be willing and able to transport clients in personal vehicle while maintaining valid driver’s license, proof of automotive insurance, and maintain a good driving record.

Responsabilidades

• Conduct outreach and engagement activities to assist individuals with symptoms of mental illness to connect with services.
• Provide case management, access transportation, advocate for clients, and visit homes.
• Provide education regarding mental illness and available services to individuals, families, or groups.
• Provide crisis intervention and/or plans for clients and their families.
• Maintain up-to-date client records and program reporting.
• Cultivate relationships with service collaborators and stakeholders.
