Qualifications

• Fluency in English and Spanish required.
• Experience in community outreach or health education preferred.
• Ability to communicate effectively with patients, families, and providers.

Responsibilities

• Conduct community outreach and education programs to promote services to patients, families, and healthcare providers.
• Build and maintain relationships with community organizations, health centers, and social services agencies to identify partnerships and referrals.
• Identify and address social, emotional, and physical needs of patients and families, and provide personalized support and resources.
• Develop and maintain understanding of services to effectively communicate offerings to the community.
• Collaborate with multidisciplinary team to develop individualized care plans.
• Conduct language-specific outreach to ensure bilingual patients receive care and support.
