AbilityFirst in Los Angeles is seeking a Community Navigator (Full-Time) to provide person-centered planning, intake coordination, and case management for participants in our licensed programs. The role emphasizes collaboration with program staff, regional center coordinators, and families to ensure individualized services aligned with goals.

You will maintain documentation, support PCP development, and engage the community through outreach and navigation activities while ensuring compliance

Formación

• High School diploma or equivalent is required.
• Bilingual Spanish/English is required.
• Completion of 3 college units in admin or related field is required.
• Bachelor’s Degree in psychology or related field preferred, or 12 ECE units or 4+ years experience.
• Must be LIC Director Qualified under Title 17/22 regulations is required.
• 2+ years of experience in developmental disability services or related field is required.

Responsabilidades

• Attend required 2-day Person-Centered Thinking training.
• Conduct screenings, phone calls and intake meetings as the first level of the intake process.
• Develop and maintain PCPs and 1-page descriptions.
• Update annual forms/documentation (emergency contacts, consent forms, seizure plans, etc.).
• Serve as liaison to Regional Center Service Coordinators and attend IPP meetings.
• Meet with participants monthly to review goals and progress; coordinate community outings.

Strong written and verbalcommunication

Driver's license

Educación

High School diploma or equivalent

3 college units in administration, business management, or staff relations

Bachelor’s Degree in psychology, sociology, or related field / 12 ECE units / 4+ years experience
