OneWorld Community Health Centers, Inc. in Omaha, Nebraska, seeks a full-time Social Worker to provide comprehensive social work services within a client-centered, multidisciplinary team.

The role includes assessment, program development, case management, and community resource access to improve patient well-being. Applicants should hold a Bachelor's in Social Work (CSWE-accredited); CSW preferred with 90-day certification timeline.

Qualifications

• Must have a Bachelor’s Degree in Social Work from a CSWE-accredited school.
• CSW by Nebraska State Law is preferred upon hiring; if not certified at hire, certification must be obtained within 90 days.
• Excellent verbal and written communication skills; English/Spanish proficiency required.
• Task-oriented and detail-oriented with excellent customer service and organizational skills.
• Proficient with Microsoft Office; willingness to learn additional programs.
• Familiarity with social service resources in the community.
• Latino culture knowledge and cultural competence.

Responsabilités

• Assess the needs of patients and families and develop appropriate programs/services.
• Investigate and determine services needed; carry out approved treatment plans; prepare case records and reports.
• Serve as a resource to clinical staff for managing psychosocial issues and accessing community resources.
• Provide direct services to individuals, families, and/or groups; assist with coping strategies and access to resources.
• Advocate for patient rights and organizational ethics; support diversity, equity, and inclusion.
• Identify and assess services to meet social, financial, and emotional needs affecting medical condition and recovery.
• Act as liaison between OneWorld and community resources; outreach to create collaborations.
• Supervise practicum students and provide training as needed.
