Qualifications

• MSW required from an accredited program.
• Licensure as a Social Worker in Nebraska and/or Iowa or eligibility.
• Strong clinical assessment and communication skills.
• Reliable transportation for home visits in the Omaha metro area.

Responsibilities

• Assess patients and families psychosocial needs in the home setting.
• Provide counseling, crisis support, and connect to community resources.
• Document evaluations, progress notes, and care plans in patient records.
• Collaborate with clinicians and other healthcare providers to coordinate care.
