Job summary

Qualifications

• Bachelor’s Degree or active enrollment in a Master of Social Work program.
• Active or enrolled in a Social Work program and will graduate within 24 months.
• Participates in mandated trainings and continuing education as directed by policies and procedures.

Responsibilities

• Assess patient needs and coordinate with post-acute care options.
• Identify interpreter needs and coordinate with department secretary.
• Assist with discharge planning and referrals to services.
• Provide on-the-job training for eligibility for Social Work position.
• Support Care Management Team with administrative tasks.
