Job summary

OneWorld Community Health Centers, Inc. in Omaha, NE seeks a Social Worker to provide integrated social services within a multidisciplinary team.

Responsibilities include assessing patient and family needs, developing programs, coordinating resources, and advocating for patients while addressing social determinants of health. Requires a CSWE-accredited BS in Social Work, English/Spanish proficiency, and strong communication, organizational, and computer skills.

Qualifications

• Bachelor’s degree in Social Work from CSWE-accredited program.
• CSW certification preferred; if not, certification required within 90 days.

Responsibilities

• Assess needs of patients and families; develop programs and services.
• Coordinate resources and services; act as liaison within integrated health care team.
• Advocate for patient rights; address social determinants of health and access to resources.
• Perform home visits as needed; maintain case records and reports.
• Supervise practicum students and provide training as needed.
