Job summary

Living Water Clinic – Clínica Agua Viva is seeking a bilingual Community Health Worker/Promotora to connect patients with healthcare services, education, resources, and programs. The role involves working outside the clinic in community events, schools, and churches, with some evenings and weekends required.

Responsibilities include engaging community members, coordinating outreach activities, and guiding patients through forms, portals, and available resources.

Qualifications

• High School Diploma or equivalent is preferred.
• CHW/Promotora certification or training is preferred.
• Bilingual English and Spanish required.
• Experience in community outreach or healthcare settings is preferred.

Responsibilities

• Represent Living Water Clinic professionally in the community.
• Coordinate community health fairs and outreach events.
• Educate community members about healthcare services in English and Spanish.
• Assist patients with forms, registrations, and healthcare portal access.
• Maintain documentation of outreach activities and referrals.
• Develop relationships with schools, churches, and community partners.
