Job summary

AltaMed is seeking Community Health Workers (CHWs) to serve as trusted members of the community, addressing chronic conditions, preventive care, and social needs. CHWs assist patients with health care navigation, referrals, and establishing routine PCP visits, while offering culturally appropriate information on chronic disease management and prevention.

They support patients facing real-life barriers to staying healthy, including social determinants of health, and undergo training in Epic,

Qualifications

• High School Diploma or GED required.
• Minimum of 2 years of experience in health care or community outreach settings.
• Bilingual English/Spanish/Mandarin/Cantonese, depending on location preferred.
• A valid BLS certification or higher per AHA or Red Cross guidelines.

Responsibilities

• Assist patients with health care navigation, appointments/referrals, and establishing routine care.
