Job summary

Zócalo Health is seeking a Community Health Worker to assist in navigating health and social needs for members in Concord, CA. The role includes enrolling patients, coordinating appointments, and connecting to community resources, with a mix of home-based and in-person work.

You will educate patients on preventive care, support care plans, and collaborate with a multidisciplinary team to promote health equity and better outcomes.

Qualifications

• Fluency in English and Spanish (written and spoken).
• High school diploma or GED required.
• Experience with CHW programs or health navigation preferred.

Responsibilities

• Assist patients with navigating health and social needs.
