Job summary

Behavioral Health Services in Gardena, CA is seeking an experienced Community Health Worker (CHW) to serve as a trusted liaison between patients, families, healthcare providers, and community resources within a Federally Qualified Health Center and/or behavioral health setting.

Under supervision, the CHW supports engagement, health education, care coordination, outreach, navigation, and social support to improve health outcomes and reduce barriers to care, while promoting trauma‑informed,

Qualifications

• High school diploma or equivalent required.
• CHW certification preferred or willingness to obtain.
• Experience in community health, behavioral health, public health or related fields preferred.
• Bilingual English/Spanish preferred.
• FQHC or community-based healthcare experience preferred.
• Knowledge of Medi‑Cal and social service systems preferred.

Responsibilities

• Provide outreach, engagement, education, advocacy, and support services to patients and community members.
• Assist patients with navigation of healthcare systems, referrals, social services, and community resources.
• Support care coordination including appointment scheduling, referral follow-up, transportation coordination, and linkage to services.
• Conduct screenings, intake activities, assessments, and patient interviews.
• Provide health education related to preventive care, chronic disease management, behavioral health, maternal-child health, wellness, and healthy lifestyles.
• Assist with registration, enrollment, consent forms, and access to services.
• Document encounters and referrals accurately in the EHR.
• Identify social determinants of health barriers and assist with resource linkage.
• Collaborate with providers and care teams to support patient care plans.
• Participate in community outreach events and health promotion initiatives.
