Job summary

AltaMed Health Services Corp is seeking compassionate Community Health Workers to join our team in Anaheim, California. In this role, you will support patients facing chronic conditions and help them navigate health care systems.

You’ll provide culturally relevant information and assist patients in securing medical appointments and resources. Ideal candidates will have a High School Diploma or GED, a minimum of 2 years of related experience, and possess a valid BLS certification.

Qualifications

• Minimum of 2 years of experience in health care or community outreach settings.
• Valid BLS certification required per AHA or Red Cross guidelines.

Responsibilities

• Assist patients in addressing social and medical problems.
• Support patients with health care navigation and referrals.
• Provide culturally appropriate health information.
