Job summary

Options for Southern Oregon seeks a Case Manager to join our mobile outreach team, providing outreach and engagement to individuals with behavioral health needs who are hesitant to engage in treatment.

The role builds rapport, assists in formulating and carrying out treatment plans, connects clients to local, state, and federal resources, and collaborates with community partners to promote access to services.

Qualifications

• Bachelor’s degree in a human services field or three years of experience providing services in a similar setting.
• Must be able to register as CADC-R or meet MHACBO eligibility requirements.
• Must be MHACBO registered or eligible to register with MHACBO.
• Proficiency with email, document creation, electronic record systems (EHR) and MS Office.

Responsibilities

• Provide outreach and engagement activities to community members who may need behavioral health services.
• Develop positive rapport with individuals to facilitate entry into treatment.
• Assist in formulating and carrying out individual treatment plans with clients.
• Help individuals access local, state and federal resources and entitlements; coordinate with community partners.
• Maintain up-to-date documentation and enter information into the EHR; handle scheduling as needed.
• Perform outreach by phone and in person to promote engagement in services; transportation of clients as required.

Education

Bachelor’s (BA/BS) degree in a human services field

Three years of experience providing services in a similar setting
