Job summary

Coastal Family Health Center in Astoria, OR is hiring a Case Manager to join our team and support residents with comprehensive, compassionate care. This full-time role offers a competitive benefits package and opportunities to collaborate with multi-disciplinary teams.

You will assess client needs, connect them with services, maintain care plans, and advocate for clients in the community. Bilingual English/Spanish speakers are encouraged to apply; relocation not required.

Qualifications

• Bachelor’s degree in a social service field is required for CHS Maternity and Infant Case Manager, or equivalent.

Responsibilities

• Build program-specific caseload by seeking referrals from providers and partners.
• Conduct intake screenings to assess clients and determine needed services.
• Conduct in-depth client interviews to determine status, barriers and needs.
• Refer clients to appropriate agencies and services.
• Assist clients with the self-referral process.
• Meet with clients regularly to monitor service plans and provide advocacy.
• Create and maintain client service plans and required documentation.
• Maintain relationships with service providers and stay informed about community resources.
• Participate in interdisciplinary team meetings and case reviews.
• Perform duties aligned with TJC/HRSA and YVFWC requirements.
• Perform other duties as needed.
