Qualifications

• Two years of scheduling experience.
• Experience in client relations, customer service, or healthcare administration preferred.
• Knowledge of home health care services and regulations preferred.
• Excellent communication and interpersonal skills.
• Strong organizational and problem-solving abilities.
• Proficiency in Microsoft Office and EHR systems.

Responsibilities

• Client Intake and Assessment: determine needs and goals for home health care services.
• Client Onboarding: coordinate onboarding for new clients and explain service offerings.
• Client Communication: serve as primary point of contact for clients and families, with regular care updates.
• Relationship Building: develop strong relationships with clients and families to ensure satisfaction.
• Care Coordination: collaborate with caregivers, nurses, and admin staff for timely care coordination.
• Quality Assurance: monitor care quality, conduct surveys, and implement improvements.
• Community Outreach: represent the agency at events to promote services and referrals.
• Documentation: maintain records of client interactions and care plans in compliance with regulations.
