Job summary

Pierce County’s Human Services Department is seeking a bilingual Vietnamese Case Manager to join our team, managing a caseload of older and disabled clients in Medicaid-related services. You will assess needs, authorize services, oversee plans, and monitor progress, with in-home client visits using a laptop.

You will work in a hybrid role, starting in-office for three months and then transitioning to telework as appropriate.

Qualifications

• Bachelor's degree in social work, gerontology, psychology, counseling and guidance, sociology or a related field.
• Two years or more of experience providing direct human services, preferably for older adults.
• Master's degree in behavioral health sciences and one year of experience in the social services field preferred.
• Bilingual and fluency in Vietnamese required; interview includes Vietnamese.
• Valid driver’s license and reliable transportation.

Responsibilities

• Assess and coordinate services for older and disabled adult clients.
• Provide formal reassessment at set intervals to gauge progress and ongoing needs.
• Develop, implement, and monitor service plans through regular contact with clients and providers; participate in case staffing.
• Travel to assigned work locations including field assessments.
