Job summary

Oregon Department of Human Services is hiring a Case Manager (Bilingual Preferred English/Spanish) to assess eligibility for long-term care services and coordinate support for older adults and people with disabilities. The role requires strong interviewing, planning, and liaison with community resources in a fast-paced state environment.

Qualifications

• Bilingual English/Spanish preferred for communication with clients.
• Bachelor's degree in Behavioral or related field or equivalent experience in human services.
• Experience with eligibility assessment and case management is ideal.

Responsibilities

• Determine long-term care program eligibility by interviewing Oregonians for healthcare, social services, and protective needs.
• Develop detailed, person-centered case plans for eligible clients.
• Conduct on-site/remote interviews using the CA/PS assessment tool on a state laptop.
• Inform clients of rights, responsibilities, service options, and recourse methods.
• Make referrals to ODHS programs and community resources.
• Process applications and inquiries within policy timeframes; documentations and decisions communicated clearly.
• Provide intake case management coordinating long-term care services per rules and plans.
• Monthly contact with clients to set up and monitor service plans and safety needs.
