Qualifications

• Bachelor's Degree in a Social Work, psychology, or a human services related field.
• Three years of experience working with at-risk youth, vulnerable populations.
• Must be bilingual (Spanish/English).

Responsibilities

• Completes intake and SPDAT assessments with clients and explains program rules and policies.
• Creates, implements and monitors an individual service plan to support goals toward self-sufficiency.
• Connects clients with social services and supports navigation of benefits and documentation.
• Conducts individual counseling sessions with clients on caseload.
• Plans and facilitates life skills, workshops and trainings for clients.
• Provides crisis intervention and on-site counseling when appropriate.
• Maintains house checks, monitors facilities and enforces safety rules.
• Maintains confidential case files and ensures timely documentation (24–48 hours).
• Participates in team meetings, trainings and community outreach; attends required trainings.
