Job summary

HAS is seeking a Case Manager to oversee a caseload of justice-involved participants, linking them to needed resources and developing individualized service plans. You’ll monitor treatment engagement, coordinate wraparound reentry services, and maintain case records in SAMMS.

Bilingual English/Spanish preferred; two years of relevant experience and a Bachelor's degree are desired. On-site position with comprehensive benefits; you will collaborate with a multidisciplinary team to improve outcomes

Qualifications

• Bachelor's degree in Human or Social Sciences, or Social Work.
• At least two years of experience in an appropriate field.
• CADC certification is preferred but not required.
• Bilingual English/Spanish preferred.

Responsibilities

• Researches and locates community and reentry resources for all participants, including legal, medical, housing, transportation, mental health, and financial resources.
• Assists in the ongoing development of individualized, person-centered service and treatment plans.
• Monitors the engagement of the clinical treatment with the multidisciplinary team.
• Links participants to needed and available community resources and provides warm handoffs to wraparound reentry services.
• Independently manages their caseload and documents all services provided in SAMMS and/or additional trackers as needed.
• Performs contingency management planning and utilization.
• Provides face-to-face reviews with the participant and/or relevant staff on a regular basis, reviewing progress and updating plans and goals as needed.
• Collaborates with the treatment team to ensure compliance and work toward improved participant outcomes.
• Provides anti-stigma education to the community and to individuals dealing with mental illness and/or substance use disorders, as well as information on prevention, treatment, and recovery.
• Assists participants in obtaining Medicaid, SNAP and other government benefits.
