Job summary

Turning Point of Central California, Inc. seeks a Personal Service Coordinator/Case Manager II to help clients access Medi-Cal, medical, housing and education services in Reedley/Kerman, CA. Strong bilingual skills in Spanish/Hmong are preferred and travel within program sites is required.

The role emphasizes careful documentation, collaboration with the PSC Supervisor, and direct client services as outlined in the service plan in a multi-site, community-based setting.

Qualifications

• Bachelor’s degree in social work or counseling or related field.
• Ability to communicate effectively in written and spoken Spanish is preferred.
• Experience with case management or related community services.

Responsibilities

• Assist in case management plan development for individuals under supervision.
• Maintain accurate case recordings per guidelines.
• Attend weekly staff and supervision meetings.
• Provide direct services per individual service plan.
• Refer clients to County Department of Mental Health and other resources.
• Respond quickly in emergencies.
• Drive agency or personal vehicle for program purposes.
• Physical presence at the program site is required.
