Job summary

Abraxas Youth and Family Services is seeking a Bilingual Case Manager to coordinate and implement intensive case management. You will supervise staff, manage workloads, and ensure documentation is complete in EHR and the UAC Portal.

Qualifications

• Associate degree or 60 credit hours from an accredited college or university.
• Six months’ experience preferred.
• Candidates with relevant experience are preferred.

Responsibilities

• Coordinates and implements case management activities.
• Oversees and distributes workload across the unit/program.
• Writes progress reports and discharge summaries as needed.
• Maintains regular contact with clients, families, and professionals.
