Abraxas Youth and Family Services seeks a Bilingual Case Manager to coordinate and oversee case management activities at our Morgantown, PA location. You will supervise staff, develop treatment plans, and ensure compliance with licensing standards while supporting youth and families in achieving independence.

The role offers paid training, weekend availability, and opportunities for growth within a compassionate team. Prior experience is valued but not required as training is provided.

Qualifikationen

• Bilingual Spanish required or preferred.
• Associate degree or 60 credit hours from an accredited college or university.
• Experience in case management or relevant social services is preferred but not required.

Aufgaben

• Coordinate and implement case management activities.
• Provide day-to-day supervision and workload distribution for case management staff.
• Develop and implement treatment plans and assessments in EHR/portal systems.
• Review and quality-check service plans for clinical soundness and compliance.
• Provide weekly individual counseling and maintain caseload.
• Maintain contact with juvenile probation officers, parents, or guardians.
