Job summary

ICAN, based in San Jose, CA, is seeking a bilingual Case Manager for the Homelessness Prevention Program. The role focuses on helping families at risk of homelessness through financial assistance, case management, and connections to community resources.

Responsibilities include intake, program enrollment, crisis intervention, and coordinating services to prevent homelessness and secure housing. Requires strong organizational skills and a growth mindset.

Qualifications

• Four-year degree in social work preferred or related degree with case management experience.
• Bilingual and bicultural in English/Vietnamese required; proficiency in English and Vietnamese.
• Strong organizational and multi-tasking abilities with limited supervision.
• Excellent communication, especially in stressful situations.
• Ability to work a flexible schedule including evenings, weekends, and holidays.

Responsibilities

• Outreach and engagement at community events and with partner agencies.
• Prescreening, intake, and eligibility assessment for program participation.
• Case management for up to 30+ households; set goals and facilitate housing stability.
• Provide referrals to medical, housing, Li-HEAP, job training, and legal services.
• Maintain confidential case files and timely HMIS documentation.
