• Provide crisis intervention and emergency support as needed.
• Monitor progress and adjust care plans; maintain detailed case notes.
• Collaborate with medical professionals, social workers, facility staff and partners.
• Deliver life-skills guidance (budgeting, job readiness, communication).
• Engage in outreach to identify and connect individuals needing case management.
