Overview: The Homeless Case Manager plays a crucial role in providing individualized supportand resources to clients experiencing homelessness. This position requires a combination ofempathy, advocacy, resource coordination, and collaboration to empower clients on theirjourney to stability and self-sufficiency.

Key Responsibilities
  1. 1. Client Assessment and Planning:

    Conduct comprehensive assessments of clients experiencing homelessness to identifytheir individual needs, challenges, and strengths.

    Develop personalized care plans in collaboration with clients, outlining specific goals andinterventions to support their journey toward stability.

  2. 2. Resource Coordination:

    Connect clients with essential resources such as housing assistance, healthcare, mentalhealth services, substance abuse treatment, employment support, and othercommunity-based services.

    Collaborate with external agencies, nonprofits, and government organizations to accessadditional resources and support.

  3. 3. Advocacy:

    Advocate on behalf of clients to navigate social services, legal systems, and communityresources.

    Assist clients in overcoming barriers and accessing entitlements and benefits they areeligible for.

  4. 4. Crisis Intervention:

    Provide immediate support during crises, such as mental health emergencies, substanceabuse issues, or sudden homelessness.Implement crisis intervention techniques and collaborate with emergency services whennecessary.

  5. 5. Regular Monitoring and Follow-up:

    Conduct regular check-ins with clients to assess progress, address emerging needs, andmodify care plans as required.

    Maintain detailed and accurate case notes, ensuring compliance with documentationstandards.

  6. 6. Collaboration with Stakeholders:

    Work collaboratively with a multidisciplinary team, including medical professionalssocial workers, facility staff, and external service providers.

    Attend case conferences, team meetings, and collaborate with community partners toenhance support networks for clients.

  7. 7. Education and Skill-building:

    Provide guidance and resources to help clients develop essential life skills, includingbudgeting, job readiness, and effective communication.

    Facilitate workshops or group sessions to address common challenges faced byindividuals experiencing homelessness.

  8. 8. Community Outreach:

    Engage in outreach efforts to identify and connect with individuals experiencinghomelessness who may benefit from case management services.

    Raise awareness about available support services and advocate for the needs of thehomeless population.

Qualifications
  1. 1. Education:

    A bachelor's or master's degree in social work, psychology, counseling, or a related fieldis typically preferred.

    Previous experience in case management, social work, or a related field, preferablyworking with individuals experiencing homelessness or in crisis situations.
