Job summary

Family Service League is seeking a full Case Manager for our Home Safe program in Bay Shore, NY. The role focuses on direct service to families involved with child protective services, tracking milestones, and coordinating care within the agency and with DSS requirements.

Responsibilities include documentation, team collaboration, and linking clients to community resources. The position requires a valid NY driver's license and prior experience with families; strong communication is essential.

Responsibilities

• Conduct linkage, coordination, advocacy and mentoring.
• Focus on goal achievement related to child protective services involvement.
• Work with clients in a culturally and linguistically responsive manner.
• Utilize Motivational Interviewing to facilitate change.
• Carry a caseload of 14 families with weekly visits.
• Document interactions and complete timely documentation.
• Collaborate with an interdisciplinary team and coordinate with program staff.
• Meet families’ individual needs and connect to community resources.
