Job summary

Family Service League is seeking a full-time Case Manager for the Alternatives for Youth Program in Bay Shore, NY. The Case Manager will coordinate in-home family support to divert involvement with the Suffolk County Probation System and will conduct home visits across the county.

Qualifications

• Bachelor's degree or higher in a human services field preferred.
• Associate's degree with one year of human service experience accepted.
• High School Diploma with three years of human service experience accepted.

Responsibilities

• Provide linkages, coordination, advocacy and mentoring.
• Engage families in conflict management and skill building.
• Provide direct services, including anger management and parenting skills to youth and parents to avoid juvenile justice involvement.
• Conduct assessments and maintain electronic health records for all services.
• Conduct home visits throughout Suffolk County.
• All other duties as assigned.

SUMMARY:

Family Service League is seeking afull-timeCase Manager for the Alternatives for Youth Program. The Case Manager will coordinate in home family support designed to divert involvement in the Suffolk County Probation System with its philosophy founded on empowering parents/guardians and equipping families for success and restoration. The Case Manager will conduct visits in the clients' home and provide direct services to the youth.

QUALIFICATIONS:

Bachelor's degree, or Associate's degree with one years of human service experience or High School Diploma with three years of human service experience required.

At least one year experience working with community resources is required.

Excellent interpersonal, organizational, and problem-solving skills required.

Strong communication skills, verbal and written, are required.

Ability to work as part of a team.

Valid and clean NYS Driver's license required.

PHYSICAL REQUIREMENTS:

This position requires sitting for long periods of time and travelling throughout Suffolk County to conduct home visits.
