Job summary

United Against Human Trafficking is seeking a Case Worker to support individuals and families toward self-sufficiency through comprehensive case management, resource coordination, and personalized support.

The role requests assessment of client needs, creation of individualized service plans, and ongoing follow-up to monitor progress toward educational, employment, financial, and personal stability goals. Office-based with occasional travel to community locations.

Qualifications

• Associate or bachelor’s degree in social work or related field preferred.
• Experience in case management, social services, or community outreach.
• Ability to maintain confidentiality and professional judgment.

Responsibilities

• Conduct client intake, assessments, and eligibility screenings.
• Develop individualized self-sufficiency plans with measurable goals.
• Provide ongoing case management and supportive counseling.
• Connect clients to housing, employment, education, healthcare, and financial assistance.
• Assist with job readiness, resumes, interviews, and referrals.
• Monitor progress and update service plans as needed.
• Maintain confidential client records and case notes.
• Collaborate with agencies and service providers to coordinate services.
• Advocate for clients to overcome barriers to self-sufficiency.
• Conduct follow-up appointments to evaluate progress.
• Prepare reports and ensure documentation meets funding requirements.
• Participate in staff meetings and professional development.
