Job summary

SCO Family of Services is seeking a Preventive Case Aide (Bilingual Spanish) in Queens, NY to support families, work with a team of Case Planners, and assist with assessments, referrals, and documentation. The role emphasizes safety, wellbeing, and community collaboration and involves on-site visits and evening hours when needed.

Responsibilities include coordinating with housing, benefits, and service providers, supporting crisis plans, and ensuring timely data entry.

Qualifications

• High school diploma or GED; Associate degree preferred.
• Previous human services experience preferred.
• Seeking bilingual candidates.
• Some evening hours required.
• Excellent organizational, written and oral skills.
• Proficient in Microsoft Office.
• Commitment to working from a strength-based perspective.

Responsibilities

• Work with Case Planners and Supervisor to assess, set goals, intervene, and manage cases.
• Conduct home and office visits and escort clients to appointments with housing, benefits, court, and other providers.
• Provide outreach, referrals, advocacy to connect families with resources and prevent eviction.
• Assist families to complete applications and documentation.
• Request collateral documentation from schools, medical providers and other service providers.
• Support safety assessments and crisis/safety plan implementation.
• Develop collaborative relationships with community resources for the program.
• Assist in running groups and child care during sessions.
• Complete documentation and data entry within deadlines; use mobile technology.
• Attend supervision, meetings and trainings.
• Provide office support: reception, filing and related tasks.
• Demonstrate sensitivity to cultural and socioeconomic diversity.
• Contribute to the program's vision and mission.
• Create and maintain a respectful learning environment.
