Job summary

Humana Inc. is hiring a Bilingual Care Management Support Assistant for a remote role serving English- and Spanish-speaking members. You will contact members by phone, perform non-clinical assessments, identify barriers to care, and help access benefits.

The shift is 11:30 AM–8:00 PM EST, with training and potential overtime in some weeks. You must document interactions clearly, support members with resources, and work in a call-center environment to support care teams.

Qualifications

• Bilingual proficiency in English and Spanish with language assessments
• At least 1 year of experience managing high-volume inbound/outbound calls
• Experience meeting documented productivity, quality, or service metrics
• Experience updating member information across multiple applications
• Proficient in Word, Excel, Teams and Outlook for daily tasks
• Willing to work an 8-hour shift including 11:30 AM–8:00 PM EST

Responsibilities

• Engage members by phone to encourage participation in care management programs.
• Complete assessments to identify health risks, needs, and follow-up opportunities.
• Assist with scheduling doctor appointments and connecting with care resources.
• Identify barriers to preventive screenings, benefits, and services.
• Support understanding and access to benefits for overall well-being.
• Manage calls in an auto-dialer and document member information
