Job summary

Humana Inc. invites applications for a Bilingual Care Coach, a remote, non-clinical member of the care management team in Puerto Rico. The role requires English/Spanish fluency and living in Puerto Rico, with telephonic engagement and data-driven care planning.

You will lead outreach, assess needs, coordinate services, and monitor progress while maintaining strict compliance. Scheduled hours are 8:30 AM–5:30 PM EST, with a 40-hour workweek and competitive benefits.

Qualifications

• Two or more years of direct adult patient care experience.
• Ability to pass a language proficiency test in English and Spanish.
• Strong communication and coordination skills within care teams.

Responsibilities

• Conduct telephonic outreach to members with chronic conditions.
• Assess needs and develop individualized care plans.
• Coordinate services with clinical and community resources.
• Identify barriers to care and advocate for member independence.
• Monitor progress and adjust care strategies as needed.
• Maintain compliance and navigate multiple computer systems.
