Job summary

POD Health in Brooklyn is seeking a Care Navigator to guide patients through enrollment and onboarding for RPM, CCM, and CHI programs. You will coordinate patient care and facilitate communication between healthcare providers and patients, verify insurance eligibility, and maintain accurate records.

You will manage referrals and leads in Salesforce, perform pre-appointment checks in ChronicCareIQ and HHA Exchange, and oversee device ordering and setup for RPM, while ensuring HIPAA compliance.

Qualifications

• High school diploma required; additional education or certification in Medical Administration preferred.
• Experience in sales, ideally in a healthcare setting.
• Proficiency with RPM and CCM platforms and EMR systems.
• Experience with Salesforce for managing referrals, leads, and patient opportunities.
• Bilingual fluency in English and Spanish required.

Responsibilities

• Guide patients through enrollment in RPM, CCM, and CHI programs.
