Job summary

POD Health in Brooklyn, NY is seeking a Care Navigator to guide patients through enrollment and onboarding for RPM, CCM, and CHI programs, focusing on underserved communities.

You will coordinate patient care, verify insurance eligibility, maintain accurate records, and work with healthcare teams to ensure timely interventions and reduced hospital visits. Proficiency with EMR systems, RPM platforms, and telehealth tools is required; bilingual Cantonese and Mandarin language skills are essential.

Qualifications

• Minimum high school diploma; medical admin certification preferred.
• Experience in healthcare sales or patient-facing roles.
• Proficient with RPM/CCM platforms and EMR systems.
• Fluent Cantonese required; Mandarin highly preferred.

Responsibilities

• Assist patients enrolling in RPM, CCM, and CHI programs with clear guidance.
• Schedule and manage patient appointments including enrollments and follow-ups.
• Collaborate with healthcare teams to coordinate RPM, CCM, CHI services.
• Ensure accurate data entry into EMR systems (e.g., AdvancedMD) and HIPAA compliant processes.
• Manage referrals, leads, and patient opportunities in Salesforce.
• Address patient inquiries and escalate issues to appropriate professionals.
• Proactively follow up with patients to ensure engagement with care plans.
• Verify insurance eligibility for RPM, CCM, CHI via platforms like e-PACES.
• Assist with device ordering and setup for RPM programs.
