Job summary

POD Health in Brooklyn, NY is seeking a Care Navigator to guide patients through enrollment and onboarding for RPM, CCM, and CHI programs. You will coordinate care, verify insurance, and maintain patient records to improve outcomes in underserved communities.

The role requires bilingual Chinese skills, experience with EMR systems like AdvancedMD, and a customer-focused, collaborative approach in a fast-paced virtual environment.

Qualifications

• High school diploma required; additional education or certification in Medical Administration is preferred.
• Experience in sales, ideally in a healthcare setting.
• Proficiency with RPM/CCM platforms and EMR systems.
• Familiarity with Salesforce for managing referrals, leads, and patient opportunities.
• Bilingual fluency in Chinese is required.
• Strong verbal and written communication skills with empathy.
• Proficiency in telehealth platforms and HIPAA compliance.

Responsibilities

• Assist patients in enrolling in RPM, CCM, and CHI programs, providing clear guidance throughout the process.
• Schedule and manage patient appointments, including enrollments, escalations, follow-ups, and healthcare visits.
• Collaborate with healthcare teams to ensure seamless coordination and delivery of care across RPM, CCM, and CHI programs.
• Ensure accurate and timely data entry into EMR systems (e.g., AdvancedMD) and other platforms, ensuring HIPAA compliance.
• Monitor and manage patient referrals, leads, and opportunities in Salesforce, maintaining accuracy and following up as needed.
• Address patient inquiries and elevate concerns related to RPM, CCM, and CHI services to appropriate healthcare professionals.
• Proactively follow up with patients to ensure engagement with their care plans.
• Conduct insurance eligibility checks via platforms such as e-PACES.
• Enhance provider efficiency by performing pre-appointment cross-checks in CCIQ and HHA Exchange.
• Maintain effective, professional communication with patients and team members via phone and email.
• Oversee device ordering and setup for RPM and educate patients on device use.
