Job summary

Pod Health in Brooklyn, NY is seeking a Care/Social Services Coordinator to support patients through our telehealth platform by linking them to community resources, behavioral health services, and care navigation. This on-site role emphasizes strong communication, case management, and precise documentation in the EHR.

You will coordinate with clinical teams to connect patients to housing, food, transportation, and financial resources, advocating to remove barriers to care while ensuring HIPAA

Qualifications

• 1-2 years experience in case management, medical social work, or patient advocacy
• Bilingual fluency in Russian
• Experience in healthcare or telehealth setting preferred
• Excellent verbal, written, and virtual communication skills
• Comfortable using EMRs and care coordination tools

Responsibilities

• Conduct virtual assessments to identify patients’ social, emotional, and practical support needs
• Collaborate with clinical teams to coordinate holistic care
• Refer and connect patients to community resources such as housing, food, transportation, behavioral health care, and financial navigation
• Advocate for patients and help remove barriers to care continuity
• Maintain documentation of assessments, referrals, and follow-ups in the EHR
• Track and follow up on referrals to ensure successful connections and outcomes
• Participate in interdisciplinary case conferences and care planning discussions
• Ensure HIPAA compliance and privacy protocols
• Maintain updated resource database
