Job summary

211 Palm Beach Treasure Coast is seeking a bilingual Care Coordinator to join our 211 Help Me Grow team. The role handles calls from parents and professionals about child development, performs screenings using ASQ, and connects families with appropriate services.

This full-time day-shift role based in Delray Beach will relocate to Lantana after 12-18 months. Benefits include health, dental, vision, EAP, life insurance, and PTO.

Qualifications

• Bachelor's degree in Social Work or related field, or an associate's degree in early child development.
• Proficiency with Microsoft Windows and ability to enter/retrieve data in databases.
• Bilingual in English and Spanish; ability to establish professional relationships.

Responsibilities

• Determine the need for services and provide information, assessment, and referrals using the 211 database.
• Complete developmental screenings and refer qualified children; maintain records of outreach and interactions.
• Link callers to Palm Beach County or Treasure Coast agencies based on needs.
• Participate in health fairs and community events to promote Help Me Grow and 211 services.
