Job summary
Fred Finch Youth & Family Services is seeking a CARE Manager for Contra Costa County to oversee case coordination, educate families, and connect clients with health benefits. Qualifications
- A minimum of three (3) years of relevant work experience.
- Bilingual- English/Spanish REQUIRED
- Reside in Contra Costa County
- High school graduate or GED.
- Long‑time resident of the community with knowledge of local resources
Responsibilities
- Oversee specific cases, coordinate health care benefits, educate and facilitate client access to care in a timely, cost‑effective manner.
- Advocate for clients as part of the interdisciplinary team and liaise with external providers to promote wellness and empowerment.
