Job summary
New York Psychotherapy and Counseling Center (NYPCC) seeks an energetic Care Coordinator to join our team at the Jackson Heights center. This full-time role supports Health Home Care clients with care planning and service coordination.
Hybrid schedule; onsite Monday, Tuesday and Thursday. You will manage an 85+ caseload and work with a multidisciplinary team to connect clients with housing, benefits, medical care, transportation, education and employment opportunities, while advocating and
Qualifications
- Bilingual (English/Spanish) required and used daily.
- Bachelor's degree required for Care Coordinator role.
- Experience with RMA and HARP services preferred.
- Strong knowledge of local community resources and networks.
- Excellent verbal and written communication with diverse clients.
Responsibilities
- Manage an 85+ caseload of Health Home Care clients.
- Assist in developing a Comprehensive Care Plan.
- Address service needs: Housing, Benefits, medical care, transportation, education, employment, crisis intervention and other services.
- Collaborate with Care Team including Supervisor, Clinicians, and Psychotherapists.
- Advocate, assess, plan, link referrals, and document in EHR.
- Engage clients monthly in person and by phone.
