Job summary

Qualifications

• AD in Human Services, Nursing or related field required.
• 2 years of experience as a Care Coordinator or Case Manager preferred in a non-profit setting.
• Fully bilingual English-Spanish (written, reading and speaking).
• Proficiency with MS Office; Salesforce experience preferred.

Responsibilities

• Answer phone calls and redirect them to the appropriate extension while providing excellent customer service.
• Prepare documentation package for the client’s initial visit, conduct the assessment and upload documents into Salesforce ensuring compliance with the agency’s regulations.
• Demonstrate interpersonal, social, cultural, and cognitive skills to think critically and effectively to best serve each client.
• Listen to clients to understand needs and develop client-driven Individual or Family Plans with short- and long-term goals.
• Refer clients to appropriate resources to assist with meeting goals and maintain professional relationships with clients to track progress.
