Job summary

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is hiring a Bilingual Care Coordinator to support Milwaukee County's CLTS program. The role requires fluency in Spanish and English and involves assessing family needs, coordinating services, and developing person-centered care plans.

You will work in a hybrid schedule, with some remote days, travel throughout Milwaukee County, and collaborate with families, providers, and internal teams to connect children with resources and ensure

Qualifications

• Bachelor's degree in a human services-related field.
• At least one year of experience working with children or individuals with developmental, physical, or behavioral health needs.
• Fluent in Spanish and English (required).
• Valid driver's license and ability to travel throughout Milwaukee County.
• Strong communication, documentation, organizational, and computer skills.

Now Hiring: Bilingual Care Coordinator – Children’s Long-Term Support (CLTS) Program

Lutheran Social Services of Wisconsin & Upper Michigan (LSS) is seeking a Bilingual Care Coordinator to join our growing Milwaukee County CLTS team. This role requires fluency in both Spanish and English.

The Children's Long-Term Support (CLTS) Waiver Program is a federally funded Medicaid program that supports children with developmental, physical, and severe emotional disabilities. As a Care Coordinator, you will partner with families to assess needs, coordinate services, develop individualized care plans, and connect children with resources that help them remain safely in their homes and communities.

Schedule: Full-time, Monday-Friday, first shift
Work Environment: Hybrid schedule with up to 2 remote workdays per week

What You'll Do
