Job summary

Humana Inc. invites applications for a Bilingual Care Coach, a remote, non-clinical member of the care management team in Puerto Rico. The role requires English/Spanish fluency and living in Puerto Rico, with telephonic engagement and data-driven care planning.

You will lead outreach, assess needs, coordinate services, and monitor progress while maintaining strict compliance. Scheduled hours are 8:30 AM–5:30 PM EST, with a 40-hour workweek and competitive benefits.

Qualifications

Job description

Become a part of our caring community. The Bilingual Care Coach works as a non-clinical member of the care management team. You will use your healthcare knowledge and employ a variety of approaches to evaluate and support a member's optimal wellness in this role. This is a remote position located in Puerto Rico, so the work is done telephonically, but you must live in Puerto Rico to be considered. Care Coaches must be fully bilingual in English and Spanish with the ability to pass a language proficiency test in both languages.

Key Responsibilities
  • You will conduct telephonic outreach (primarily via auto-dialer) to members with multiple chronic conditions and financial or functional barriers.
  • You will assess member needs through data review, structured assessments, and direct conversation to develop and manage individualized care plans.
  • You will coordinate services and connect members to appropriate clinical and community resources to support health outcomes.
  • You will identify and resolve barriers that impede effective care and member independence.
  • You will monitor progress toward desired health outcomes and adjust care strategies as appropriate.
  • The Care Coach maintains strict adherence to compliance requirements and navigates multiple computer systems simultaneously while engaging members in a structured environment with limited schedule flexibility.

Scheduled Hours

The shift is 8:30 AM to 5:30 PM EST (9:30 AM to 6:30 PM during Daylight Savings). No time off is permitted during the first month of training. The first 90 days following training are critical in building your foundation; therefore, planned or unplanned time away should be minimized as much as possible.

Required Qualifications
  • You must reside in Puerto Rico for this position.
  • Associate degree in any subject (No ADN, RN, LPN or SW degrees).
  • Must be bilingual in English and Spanish (reading, writing and speaking).
  • 2 or more years of experience in direct adult patient care. (This experience can be in settings such as long‑term care, home and community-based services, hospital/facility setting, case management, or managed care organizations.)
  • Experience working with members of a healthcare team.
  • Experience navigating a variety of electronic software programs, including electronic medical records.
  • Experience building relationships with internal and external customers.
Preferred Qualifications
  • Bachelor's degree in an Allied Health Science.
  • Experience working with elderly or chronically ill patients.
  • Experience with Medicare and Medicaid patients.
  • Experience conducting health assessments with patients.
  • Experience assisting patients with health benefits and coordinating care.
  • Experience with health promotion, coaching and wellness.
  • Knowledge of community health and social service agencies and additional community resources.
Work at Home Requirements

To ensure Home or Hybrid Home/Office employees’ ability to work effectively, the self‑provided internet service of Home or Hybrid Home/Office employees must meet the following criteria: At minimum, a download speed of 25Mbps and an upload speed of 10Mbps is required; wireless, wired cable or DSL connection is suggested. In certain roles, the minimum recommended internet speed required by Humana may not be sufficient for business needs. Humana reserves the right to require associates to upgrade their internet service if necessary. Work from a dedicated space lacking ongoing interruptions to protect member PHI / HIPAA information.

Travel

While this is a remote position, occasional travel to Humana's offices for training or meetings may be required.
