Job summary

Wider Circle seeks a Care Advocate to join our nationwide team, supporting our WiderCare+ care delivery program and CHI/PIN initiatives. You will collaborate with clinicians and operations to build trust, promote health, and bridge clinical care with social needs.

Ideal candidates have 3+ years in community outreach or healthcare, with CHW training preferred. Strong computer skills, reliable transportation, and flexibility for after-hours work are required.

Qualifications

• 3+ years of community outreach, facilitation or healthcare experience
• CHW training or certification preferred
• Willing to complete CMS-mandated training and state requirements

Responsibilities

• Execute person-centered PIN care plans for high-risk patients
• Bridge clinical care and billing by coordinating provider availability
• Assess social needs (SDOH) and set targeted action plans
• Help patients navigate the New Jersey healthcare landscape
• Coordinate with the tech team to update CMS with accuracy
• Refer patients to community resources and maintain partnerships
• Provide education and emotional support to patients and families
• Maintain logs and ensure CMS documentation compliance
