Job summary

Wider Circle in Texas is seeking a Care Advocate to join the remote Care Delivery team. You will support WiderCare+ and PIN programs, building trust with patients while coordinating care across clinical and social services.

The role emphasizes SDOH assessment, system navigation, and documentation in the Case Management System. You will work with clinicians, teach self-advocacy, and refer patients to local resources with a patient-centered approach.

Qualifications

• Hold an active CHW certification in Texas.
• Bilingual English and Spanish required.
• 3+ years of relevant community outreach or healthcare experience.
• Valid driver's license and reliable transportation.
• Willing to complete CMS training and state requirements.
• Strong computer skills and ability to navigate web/app systems.
• Understand socio-economic and public health challenges in Texas.
• Excellent written and verbal communication.

Responsibilities

• Execute PIN care plans for high-risk patients.
• Bridge clinical care with billing practitioners and manage scheduling.
• Assess social needs and develop action plans.
• Assist patients in navigating Texas healthcare landscape.
• Coordinate with tech team to update CMS patient charts.
• Refer patients to local resources and coordinate partnerships.
• Provide motivational interviewing and education to improve adherence.
• Maintain detailed logs for Medicare integration services.
