Job summary

Wider Circle is seeking a Care Advocate to support the care delivery program (WiderCare+) and drive CHI and PIN initiatives. You will work with clinicians and operations to build trust, educate patients, and bridge clinical care with social needs.

The role emphasizes community outreach, CMS documentation, and the ability to work remotely within a highly independent, field-focused team, with travel within a 50-mile radius for home visits.

Qualifications

• 3+ years of relevant community outreach, facilitation, or healthcare experience.
• CHW training or certification preferred; equivalent experience considered.
• High school diploma or GED; college degree preferred.
• Valid driver’s license, reliable transportation, flexible schedule for occasional after-hours work.

Responsibilities

• Execute person-centered care plans for high-risk patients.
• Bridge communication between patients and billing practitioners; support care coordination.
• Assess social determinants of health and develop action plans to address unmet needs.
• Assist patients in navigating the New Jersey healthcare landscape and coordinate transportation or telehealth access.
• Update patient charts in the Case Management System with accuracy.
