Job summary

Merakey is seeking a Blended Case Manager to join our Behavioral Health program in Lewistown, PA. The BCM will provide community-based services to children, adolescents, and adults with behavioral health needs, often visiting clients at home or in community settings.

The role involves coordinating services, developing individualized plans, and advocating for clients. Local counties include Juniata, Huntingdon, and Mifflin, with opportunities for professional growth and BHHEP program benefits.

Qualifications

• Must have either a bachelor’s degree in social sciences, or be a Registered Nurse, or have a high school diploma with 12 college credits in social sciences and two years of relevant experience.

Responsibilities

• Help consumers build a network of community resources and supports.
• Link consumers with needed services in their community.
• Advocate for consumers and families and develop an individualized service plan with measurable goals.
• Navigate complex systems such as special education, social services, and housing.
• Find access to basic living needs and skills (employment, housing, food, medical care).
• Provide support for on-call crisis intervention.

• Possess a Bachelor’s degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education


• OR be a Registered Nurse


• OR possess a high school diploma AND 12 semester college credit hours in sociology, social welfare, psychology, gerontology, or other social science AND two years' experience in direct contact with mental health consumers required for this level.


• Must possess an active and unrestricted driver's license and access to reliable transportation.


• Bilingual proficiency in Spanish is preferred but not required.

Schedule

Primary hours are Monday-Friday, 9 am-5 pm; Occasional early evening or weekend on-call coverage may be required.

Earn

$19.97/hour or $21.97/hour with the selection of Enhanced Pay.

Bilingual

Bilingual candidates (Spanish-English) are strongly preferred and may be eligible for a higher starting hourly rate.

Position Details

The Blended Case Manager (BCM) provides community-based services in Juniata, Huntingdon, and Mifflin Counties, assisting children, adolescents, and adults with a variety of behavioral health, medical, and community-living needs. The majority of time will be seeing consumers in their homes or in other community settings, with designated times spent in the Lewistown office.
