Eliot Community Human Services, Inc. in Lynn, MA is seeking a Behavioral Health Services Manager to support the Collaborative Care Model (CoCM) within primary care and community settings.

The role combines clinical oversight with care coordination and travel to serve clients across Lynn and surrounding areas. The ideal candidate holds a Master’s or Bachelor’s with significant experience in behavioral health care management, will supervise treatment plans, provide evidence-based interventions,

Qualifications

• Master’s degree in human services or Bachelor’s degree in related field with at least five years experience.
• Experience in behavioral health care management, case management, or a related clinical role.
• Must have a valid driver’s license and reliable transportation for providing services in the community.

Responsibilities

• Functions as part of the primary care team and works under clinical supervision of the PCP.
• Develops and implements individualized care plans with patient, family, and collaterals.
• Provides evidence-based behavioral interventions including behavioral activation, problem-solving, and motivational interviewing.
• Delivers side-by-side interventions focused on problem-solving, skills training, and behavioral modeling.
• Tracks treatment response and monitors patients for changes in symptoms or treatment side effects.
• Ensures patient safety through crisis intervention as needed.
• Promotes community integration by linking with community resources and reducing stigma.
