Qualifications

• Master’s degree in human services or Bachelor’s degree in related field with at least five years experience.
• Experience in behavioral health care management, case management, or a related clinical role.
• Must have a valid driver’s license and reliable transportation for providing services in the community.

Responsibilities

• Functions as part of the primary care team and works under clinical supervision.
• Develops and implements individualized care plans in collaboration with patient and family.
• Provides evidence-based behavioral interventions including behavioral activation, problem-solving treatment, and motivational interviewing.
• Delivers side-by-side interventions focused on skills training and encouragement as per treatment plan.
• Tracks treatment response and monitors symptoms, side effects, or complications.
• Ensures patient safety through crisis intervention as needed.
• Promotes community integration and helps access community resources.
