Job summary

Eliot Community Human Services is looking for a dedicated Behavioral Health Services Manager to foster integrated care in Boston, MA. This role involves collaborating with the primary care team and implementing tailored care plans to support patients’ mental health recovery.

The ideal candidate will have experience in behavioral health management, a Master’s degree, and be bilingual in Spanish and English. This position requires travel to provide community-based support across multiple service areas.

Qualifications

• Master’s degree in human services or Bachelor’s degree with at least five years experience.
• Experience in behavioral health care management, case management, or a related clinical role.
• Valid driver’s license and reliable transportation for community services.

Responsibilities

• Function as part of the primary care team under supervision of the PCP.
• Develop and implement Individualized Care Plans in collaboration with the patient.
• Provide evidence-based behavioral interventions including problem-solving treatment.
