Job summary

ProCare Hospice of Nevada seeks a Bereavement Coordinator to lead grief counseling and manage the hospice bereavement program for pre- and post-death care. You will supervise spiritual counseling within an IDG framework and ensure compliance with federal and state regulations and ProCare policies.

The role includes coordinating bereavement services, conducting assessments, planning care, and delivering outreach, groups, and memorial services for patients, families, and staff.

Qualifications

• Documented experience or education in grief or loss counseling sufficient to supervise the hospice bereavement program
• Must identify and address spiritual and bereavement needs of hospice patients, families, and caregivers
• Familiar with Medicare Hospice Conditions of Participation and state regulations

Responsibilities

• Fosters effective relationships with coworkers, patients and community professionals
• Participates in IDG meetings and contributes bereavement assessment findings
• Performs initial bereavement assessments and incorporates findings into care plans
• Provides pre-death grief support and anticipatory grief counseling with the social worker and IDG
• Develops bereavement plans of care after patient death and documents care delivery
• Delivers bereavement services to families and staff as identified in the plan of care
• Coordinates bereavement outreach, support groups and memorial services
• Oversees bereavement program compliance and maintains documentation
